All information submitted is confidential and intended for the sole use of Glass Mosaic Canada. Complete this form, and return it to Glass Mosaic Canada along with:
- Your business card
- A copy of your Provincial tax license if you are an Ontario based merchant (if your province does not
have a Provincial sales tax, a copy of your Federal or Provincial business registration may be
substituted)
- If you want a Sample Book, you can order it as usual, and we will refund $24.99 to your first order of
$ 300.00
